I have a cloud postgresql database that i connect to hosted by Neon. If I upgrade to the hacker subscription, do I need to enable the postgres server switch in order to be able to use that private cloud connection? Or will just the $5 work for me and is that postgres server only talking about the pythonanywhere hosted one?

No you wont need to switch on Postgres in PythonAnywhere. Doing that will give you a hosted postgres server in PythonAnywhere
